   arrive because all other social services depend on their having proof that

4.    School Registration     
The child must have the same documents no matter which school district he/she is in. The child must be accompanied by the parent and bring:
·        Proof of Age : form I-94 (client receives this on entering US)
·        Proof of residency such as a letter or a utility bill to the legal resident. If the family is living in an apartment, they must bring additional documents. 
·        Immunization records, especially PPD test results and chest x-rays
·        Completed school application form

Make the Appointment for the Physical or Dental check up as soon as possible. When the client applies for County Assistance, s/he is also applying for a temporary ACCESS card, which will come in the mail in a few days. The ACCESS card is good until the client’s HMO gets entered into the system. At the time of the appointment at County Assistance, the case worker will ask the client to choose an HMO. The client must choose an HMO which is convenient, otherwise the HMO will be assigned. The adults must go to the health clinic for a physical and for any follow up on physical problems. The children will need inoculations before registering for school.                              
 
        When the client gets the ACCESS card in the mail, s/he must go to the appointment at the health clinic right away in order to meet the 30-day requirement for having a physical. Also the ACCESS card will no longer be valid, once the client has gotten assigned to an HMO. 
 
The client must take:
·        The health referral letter from LCFS (client gets this at orientation)
·        The health screening protocol letter from LCFS (client gets this at orientation)
·        The ACCESS card
·        Personal health records and history in the IOM bag.
 
IV.              School Registration

V.                PA State ID
All adults must have a PA State ID. When the adults receive Social Security Cards we will write a referral letter for each adult. They will go to the nearest DOT office and get a 4-year State ID. They need the following documentation:
    I-94
    Social Security Card
    Employment Authorization Card or Reception and Placement Form
    2 poofs of residency. (Referral letter from LCFS is one proof.)
 
PA Drivers License
1.Take written test at DOT, receive driver’s permit
If the client speaks Russian, we have the driver’s manual translated
When the client is ready to take the test, call Todd Miller (215) 747 7500 x257. He will send out the practice test in Russian. If the client passes the test, then the client can go to DOT with an approved interpreter and take the written test.
2. Make an appointment to take the road test. 
3. Receive the Drivers License.

Step III.    Health Screening

o                   Find out the address of the client and find the nearest
            Health center
·               Get the names and birthdates of each client
·               Use the Health Appointment Forms in the top drawer of the cabinet in the Health Section in a folder marked Health Appointment Forms
        The client needs the ACCESS card for the appointment. They must call the health center or us to reschedule the appointment.
The client must take:
·         The health referral letter from LCFS (client gets this at orientation)
·         The health screening protocol letter from LCFS (client gets this at orientation)
·         The ACCESS card (Client gets this in the mail)
·         Personal health records and history found in the IOM bag.
 
All children of school age need to get their inoculations before registering for school. They can walk into the Health Centers once they have applied for health benefits. They must go early!
          
IV.       School Registration
 In the Welcome Box you will find papers called Registering a Child in the Philadelphia Schools. There is a HOTLINE (215) 299-3688 or (215) 299-7672 to call and they will tell the client where the child is to go to register, depending on the zip code of the child’s residence.
        The child must be accompanied by the parent and needs to bring:
·         Proof of Age : form I-94 (client receives this on entering US)
·         Proof of residency such as a letter or a utility bill to the legal resident. If the family is living in an apartment, they must bring additional documents. See Registration Requirements Packet

            SEAMAAC provides:
            ~Services for families with children ages 6 months to 6 years.
            ~Outreach workers that go out to homes. The outreach workers will test the children for lead, and will perform a risk assessment on the home.
           ~Education for the family on the hazards of lead, and lead safety.
